If you would like to purchase a plaque, please complete the PDF form below and email it to vhf@virtua.org or mail it to the address below with a check for payment. If you would like to use a card for your purchase, please call us at 856-355-0830.
Checks should be made payable to Virtua Health Foundation and mailed to 303 Lippincott Drive, 4th Floor, Marlton, NJ 08053.
